The number one cause is certainly corrosion around the reinforcing steel within the concrete. This problem continues to affect the structure because rust can occupy ten times the volume of steel, creating tensile stresses and eventually cracking and spalling. This is a huge liabilty because the concrete will begin to delaminate, break and literally fall off.
